Risk Assessment and Management

We will evaluate your loved one’s home to identify and help manage potential risks based on their current physical ability and health status. This could range from recommending home modifications to prevent falls and facilitate self-care, to advising on the safe use of mobility aids. Our goal is to work in conjunction with you and your loved one to create a safe, supportive environment that minimizes risks and promotes healing and well-being.

Customized Care Plan

The key to a successful transition to home care is a carefully designed care plan. Our team collaborates with you to create a tailored care plan that addresses your loved one’s specific needs. We monitor progress continuously, adjust the care plan as needed, and provide timely feedback to all involved in the care process.
